Some of them reportedly involve alleged payments, rebates or refunds related to drugs or other health services, apparently aimed at obtaining credit card information.  The Minnesota Hospital Association urges Minnesotans to exercise caution in any unsolicited communication regarding their health care. Patients are suffering as a result.  State funded Medicaid reimbursements are not covering the cost of care for beneficiaries. State promises for mental, maternal and children’s health care are not being fulfilled. Basic needs, like emergency medical services, are being neglected.  This translates to service cutbacks that providers are being forced to make, like labor and delivery closures; it means people spend hours waiting for care in emergency departments; it means patients spend days in hospital hallways waiting for inpatient beds; it means kids spend days, even weeks, waiting in hospitals for mental health treatment that may never come. It’s part of the reason paramedic responses have stretched out to more than an hour in some parts of the state. It’s part of the reason why health systems have been forced to eliminate hundreds of jobs in the last year.  Underpayments and non-payment for public programs meant Minnesota hospitals provided more than $2 billion in unpaid care in 2023 alone. Nearly a third of that care, $800 million, was just for beneficiaries of state-sponsored Medicaid programs. Hospitals can’t afford to keep that up.  Minnesota can’t keep passing its responsibilities off to the state’s not-for-profit health care providers. A financial surplus is no benefit if it comes at the expense of Minnesotans’ well-being. Minnesota needs to act now. And with a $3.7 billion projected budget surplus, it CAN act now to address the real deficit the state faces – a generational threat to patient care and Minnesota’s health.  Austin Lozano Mon, 04 Mar unnecessary and unpaid, threatening access to care for patients across Minnesota.  Jan. 31, 2024, Saint Paul, Minn. -- A new comprehensive survey of patient care in Minnesota hospitals has found persistent, ongoing delays in discharges from emergency departments and inpatient care, resulting in tens of thousands of days of unnecessary hospital-level patient care and staggering financial losses.  “Minnesota hospitals have gone from being a safety net, to being a catch-all for patient care,” said Minnesota Hospital Association CEO and president Dr. Rahul Koranne. “This is a function they were never intended for, can’t afford, and isn’t good for patients. This gridlock is preventing Minnesotans from getting care that their lives depend on. Policy makers must act.”  The survey of 101 hospitals by the Minnesota Hospital Association follows similar data collection by the Minnesota Department of Human Services in the first five months of 2023, which found more than 76,245 days of unnecessary hospital care. The new survey found 65,555 additional days of unnecessary patient stays June through October. (The new data reflects usual seasonal variation in hospital care, as well as changes in administrative data collection.) These surveys represent an annual total of nearly 195,000 patient days of avoidable and unpaid care. This patient gridlock not only reduces overall capacity for hospital care; it also cost Minnesota hospitals and health systems an estimated $487 million in unpaid care.  These delays include patients stuck in hospital beds waiting for transfers to nursing homes, rehabilitation units, mental health treatment facilities, and other sub-acute care facilities. The latest survey also found hospitals provided 9,223 days of emergency department stays, often people stuck waiting for inpatient care, or simply brought to a hospital for lack of any alternative. These stays increased waits for other patients who need care and forced some patients to find other care elsewhere, with potentially life-altering delay. This unsustainable level of unpaid care is a root cause of financial distress for hospitals all over the country, combined with payers that don’t cover the full costs of care, and fast-rising costs. In the first half of 2023, an alarming 67% of Minnesota hospitals reported operating losses. Elsewhere, two hospitals have already closed in western Wisconsin in 2024, and 12 hospitals around the U.S. entered bankruptcy last year, more than the previous three years combined.  Koranne noted the Minnesota Department of Health has already had three hearings on hospital service closures in January – half of the total for all of last year in Minnesota. "We are at a critical point,” Koranne said. “Our hospitals are facing immense financial pressure. by more than $190 million for the first six months of 2023 in latest analysis of hospital and health system financial data, with average losses nearly doubling from 2022.     Nov. 13, 2023, Saint Paul, Minn. – A new Minnesota Hospital Association (MHA) analysis of hospital and health system finances shows losses growing at many of the state’s major health care providers. Data collected from MHA members show ongoing financial losses nearly tripled in some cases. Key findings include: Median hospital and health system operating margins declined from -.5% in 2022 to -2.7% in the first half of 2023. 67% of hospitals and health systems in the MHA analysis had negative operating margins, which means they were losing money. This is up from 55% of hospitals and health systems that had negative margins in 2022. The costs of labor grew by 7%, and supply and service costs grew by 6%. Nearly a quarter of member hospitals and health systems reported labor costs rising by double digit percentage points, and a third of hospitals said supply and service costs had risen by more than 10% over 2022.     The losses also correspond to a rise from 62 percent to 64 percent in the proportion of Medicare and Medicaid patients reported by nearly every hospital. Those programs continue to reimburse providers well below the actual cost of providing care, routinely underpaying an estimated 27% below cost for Medicaid, and 20% below cost for Medicare.     "Workforce shortages, cuts in the federal 340B Drug Pricing Program, persistent delays in discharging patients to step-down care, and a crisis in behavioral and mental health care are adding to the grave financial challenges hospitals and health systems are facing,” says MHA President and CEO Rahul Koranne.      Similar pressures have closed programs and cut care at more than 50 hospitals across the country already in 2023 and hundreds more could follow. Nearly 30% of U.S. rural hospitals are facing closure, according to the Center for Healthcare Quality and Payment Reform, and that could include as many as a half dozen in Minnesota if urgent action is not taken.     About the Data: The non-public financial data referenced here is provided voluntarily by hospitals and health systems in Minnesota. As mental health challenges are on the rise for young patients in all communities, connecting with cultural practices can help promote healing. Change to Chill is a collection of resources supported by Allina Health and the Indigenous People’s Task Force to help indigenous and native youth navigate challenges and reduce stress in a culturally responsive way.   Resources include healing from stress and anxiety using the four sacred medicines, mindfulness, dealing with grief and loss, and more.
