Supporting the health of our communities means more than providing care in hospitals and clinics. M Health Fairview is addressing social influencers of health for residents of the East Side of St. Paul by connecting them to resources and community partners related to housing, food security, mental health support, COVID-19 response, faith-based and cultural resources, and more. M Health Fairview funds this work through the
Eastside Health and Wellbeing Collaborative, a collaboration between community organizations working to advance health equity in the east metro.
The Cultural Broker program is one way the Eastside Health and Wellbeing Collaborative supports equity. Cultural Brokers – M Health Fairview staff working at community partner sites – help participants navigate systems including health care and schools and connect them to resources. By bridging cultural gaps, Cultural Brokers help address health disparities in their communities.
