Hospital Improvement Innovation Network
The Partnership for Patients (PfP)
is a nationwide public-private collaboration that began in 2011 to reduce
preventable hospital-acquired conditions and readmissions. MHA has been
involved with PfP since its inception, serving as a national leader and
significantly reducing hospital-acquired conditions. Building on more than 15
years of quality improvement work at MHA, hospitals and health systems across
the state have made significant progress toward reducing hospital-acquired
conditions and readmissions, as well as reducing harm across the board.
From 2011-16, MHA was selected for both the first round and second round of Hospital Engagement Network (HEN) funding
awarded by PfP. From 2016-19, MHA was selected
to receive Hospital Improvement Innovation Network (HIIN) funding awarded
by PfP. Through 2019, the MHA HIIN will work to achieve a 20 percent decrease
in overall patient harm and a 12 percent reduction in 30-day hospital
readmissions.
Efforts to address health equity for Medicare beneficiaries are central to HIIN
efforts.
